Services include hands-on assistance, prompting, reminders, supervision, and health monitoring.
The Indiana Family Supports Waiver exclusively covers PAC services.
Residential Habilitation & Support (RHS) – Indiana
Residential Habilitation & Support in Indiana follows the Person-Centered Individualized Support Plan.
RHS services build skills for independent living within Indiana communities.
Supports include daily living assistance, transportation, adult education, and social skill development.
Day Habilitation (DHI/DHGS) – Indiana
Indiana Day Habilitation supports communication, self-care, and community living skills.
Services encourage socialization, relationship building, and natural supports.
Indiana FSSA and DDRS approved providers deliver services in community or facility-based settings.
Services operate outside private residences and support one-to-one or group participation.
